PROGRAM COORDINATOR POSITION

Our Program Coordinators have an assigned caseload of persons receiving various hourly services in Anoka, Hennepin, Ramsey, and Isanti Counties, and office out of Fridley. We are looking for a professional who is driven to lead highly successful direct support teams in providing person-centered supports and services. The ideal candidate will be an organized skilled professional who understands that people with disabilities deserve opportunities to gain skills or supports needed to live life to the fullest. Program Coordinators play a vital role within our agency and have a critical impact on the lives of the people we support.

Full-time - 35-40 hours per week with some evening or weekend hours, when needed.

Starting Rate of Pay: $15.50 - $17.50/Hr

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Designated Program Coordination and Evaluation. Develops and coordinates person-centered plans that ensure quality services and supports including oversight, instruction and evaluation of the effectiveness of service delivery
  • Supervises Direct Support Professionals. Participates in recruitment activities such as interviewing, selection, and training direct support professionals as well as mentoring, on going support and employee development.
  • Manages Fiscal Responsibilities. Ensures employee’s time is accurate and used effectively to support persons served. Ensures proper billing and utilization of authorized service hours.
  • Effectively communicates In a professional manner within and outside of Life By Design, Inc.
  • Professionalism and Leadership. Represents Life By Design in a positive professional manner while consistently promoting the agency’s mission, vision and goals.
  • Presents information, resources and concepts to team members that encourages and supports self determination.
  • Successfully participates in internal and external meetings that may involve negotiation, compromise and discussing sensitive topics.
  • Develops or maintains positive working relationships with a person’s supported, families, county and state officials, professionals, as well employees and agency colleagues.
  • Effectively manages time and independently completes responsibilities in a timely manner.
  • Identifies issues or potential issues and offers viable options to correct or improve situations, practices, processes for the agency and for the opportunities, safety and well being both the employee and persons supported.
  • Ensures compliance with applicable rules, regulations, and Life By Design, Inc. policies and procedures, and practices.

Other Duties and Responsibilities:
The program coordinator is responsible for coordinating services for assigned programs and to help develop and maintain a well trained, professional and flexible work force that meets the service needs required by the person served. The Program Coordinator often chooses to temporarily provide planned direct support to establish a relationship, assess skills or generate program ideas. Will be skilled at using employee resources to fill open shifts, however must be willing to step in when needed or appropriate to provide temporary or emergency direct support.

Skills, Knowledge, Credentials:
Credentials and experience related to “Designated Coordinator” as defined in Rule 245D

  • Bachelor degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
  • Associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
  • a diploma in a field related to human services from an accredited post secondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age and older.
  • Supervisory experience (preferred)
  • Effective written and oral communication skills
  • Proficient computer skills including Microsoft Outlook, Word and Excel.
  • Ability to work with databases is a plus.
  • Ability to effectively manage multiple and varied duties;
  • Ability to work with numbers, math, and spreadsheets.

Other Employment Requirements:
Must be at least 21 years of age, have a valid Minnesota Drivers License and able pass a Driver’s Record Check, and an OIG Check; be able to provide proof of current car insurance, and have effective written and oral communication skills.
